019K Trauma with Contusion and Vascular Injury

12 year old boy with Hematuria Following Blunt Trauma to the Right Kidney associated with Contusions and a Fracture of the Transverse Process

CT – Blunt Trauma Renal Contusion
12-year-old male with hematuria and flank pain following blunt trauma to the right flank after MVA. Axial CT through the kidneys shows a delayed nephrogram and multiple ill-defined filling defects in the right kidney consistent with contusions and possible vascular injury. An associated fracture of a right transverse process is noted with swelling of the ipsilateral erector spinae muscle and psoas muscle at that level.

Right Kidney Contusions, Wedge Shaped Perfusion Defects and  Fracture of the Transverse Process

CT – Blunt Trauma, Delayed Nephrogram and Perfusion Defects Question Vascular Injury and Vasospasm
12-year-old male with hematuria and flank pain following blunt trauma to the right flank after MVA. Axial CT through the kidneys shows delayed nephrogram and multiple wedge-shaped perfusion defects in the right kidney consistent with a vascular injury possibly related to vasospasm. An associated fracture of a right transverse process is suggested with swelling of the ipsilateral erector spinae muscle and psoas muscle at that level.

Right Kidney Wedge Shaped Perfusion Defects and  Swelling of the Psoas and Erector Spinae

There is swelling of the ipsilateral erector spinae muscle and psoas muscle.

CT – Blunt Trauma, Delayed Nephrogram and Perfusion Defects Question Vascular Injury and Vasospasm
12-year-old male with hematuria and flank pain following blunt trauma to the right flank after MVA. Coronal CT through the kidneys shows delayed nephrogram and multiple wedge-shaped perfusion defects in the right kidney consistent with a vascular injury possibly related to vasospasm. There is swelling of the ipsilateral erector spinae muscle and psoas muscle.

Right Renal Artery Defect Question Vascular Injury

CT – Blunt Trauma, Question Vascular Injury and Vasospasm
12-year-old male with hematuria and flank pain following blunt trauma to the right flank after MVA. Coronal CT through the abdominal aorta and proximal right renal artery shows a focal defect in the artery raising the possibility of a vascular injury.

Small Right Renal Artery – Question Vasospasm

CT – Blunt Trauma, Question Vascular Injury and Question Vasospasm
12-year-old male with hematuria and flank pain following blunt trauma to the right flank after MVA. Coronal CT through the right hilum shows an artery (red arrowheads) and vein (blue arrowhead) in the renal hilum. The apparent small size of the vessel raises the question of vasospasm in the clinical context and other radiological findings of multiple wedge-shaped perfusion defects throughout the right kidney. No follow up imaging was available
