Emphysematous Pyelitis

  • Emphysematous pyelitis (EPN) is
  • uncommon form of acute pyelonephritis
  • caused by gas-forming bacteria
    • typically caused by enteric gram-negative facultative anaerobes.
      • Escherichia coli is isolated in 66% of patients, and
      • Klebsiella species are reported in 26% of patients.
      • Proteus,Pseudomonas, and Streptococcus species are other organisms

78-year-old female with chronically obstructed left kidney presents with flank pain and fever. CT shows air in the hydronephrotic left renal pelvis with associated post obstructive renal atrophy and thinning of the cortex. There is perinephric induration and thickening of Gerota’s fascia. Regional lymphadenopathy is present in the retroperitoneum
